摘   要：目的  探討PDCA循環(huán)的使用在提高病案首頁(yè)信息質(zhì)量中的應(yīng)用價(jià)值。方法  選取300例實(shí)施PDCA循環(huán)的階段性改進(jìn)，即進(jìn)行計(jì)劃（P）、實(shí)施（D）、查核（C）、行動(dòng)（A）循環(huán)的病案首頁(yè)資料為實(shí)驗(yàn)組，同時(shí)選取同時(shí)期本院未使用PDCA循環(huán)模式300例患者為對(duì)照組，比較兩組病案首頁(yè)信息缺陷及書(shū)寫(xiě)的錯(cuò)誤發(fā)生率。結(jié)果  實(shí)驗(yàn)組病案首頁(yè)信息缺陷率（3.00%）低于對(duì)照組（22.67%），差異有統(tǒng)計(jì)學(xué)意義（P<0.05）;實(shí)驗(yàn)組病案首頁(yè)信息書(shū)寫(xiě)錯(cuò)誤發(fā)生率（1.00%）顯著低于對(duì)照組（28.00%），差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論  應(yīng)用PDCA循環(huán)有利于改善病案首頁(yè)信息，提高病案信息質(zhì)量，在醫(yī)療質(zhì)量的改進(jìn)中起到積極的作用。

　　Abstract：Objective  To explore the application value of PDCA circulation in improving the information quality of the first page of the medical record.Methods  A total of 300 cases of PDCA cycle improvement were selected，that is，the first-line data of the planning （P），implementation（D），check（C），and action（A）cycles were taken as the experimental group.The 300 patients with PDCA circulation model were the control group.The information defect and writing error rate were compared between the two groups.Results  The defect rate of information on the front page of the medical record in the experimental group（3.00%）was lower than that in the control group （22.67%），the difference was statistically significant（P<0.05），and the incidence of information error in the front page of the experimental group（1.00%）was significantly lower than that in the control group（28.00%），the difference was significant（P<0.05）. Conclusion  The PDCA cycle is helpful to improve the home page information of the medical record.Improving the quality of medical record information plays an active role in the improvement of medical quality.

　　PDCA循環(huán)由美國(guó)質(zhì)量管理專家提出的一種按照計(jì)劃（P）、實(shí)施（D）、查核（C）、行動(dòng)（A）四個(gè)階段進(jìn)行循環(huán)管理的一種方法，廣泛應(yīng)用于醫(yī)院質(zhì)量管理并取得了良好的效益[1]。病案首頁(yè)是病案的窗口，用于評(píng)估入院患者疾病情況的載體，其信息質(zhì)量不僅反映醫(yī)院醫(yī)療決策的水平，還對(duì)臨床治療具有重要的指導(dǎo)價(jià)值。因此，提高病案首頁(yè)信息質(zhì)量，有利于促進(jìn)醫(yī)院的長(zhǎng)遠(yuǎn)發(fā)展。本文針對(duì)病案首頁(yè)資料的質(zhì)量問(wèn)題，運(yùn)用PDCA循環(huán)理論進(jìn)行質(zhì)量跟蹤，以滿足醫(yī)院及上級(jí)主管部門對(duì)病案首頁(yè)的需求。
　　1資料與方法
　　1.1一般資料  選取我院2016年1月～12月實(shí)施PDCA循環(huán)前后300份病案首頁(yè)信息資料作為實(shí)驗(yàn)組，男性病例190例，女性病例110例，年齡23～45歲，平均年齡（32.41±3.20）歲。同時(shí)選取同時(shí)期未實(shí)施PDCA循環(huán)300例病案首頁(yè)為對(duì)照組，其中男性病例120例，女性病例180例，年齡25～50歲，平均年齡（31.61±2.83）歲。兩組患者性別、年齡等一般資料相比，差異無(wú)統(tǒng)計(jì)學(xué)意義（P>0.05），有可比性。
