【摘要】 目的：研究微創(chuàng)股骨近端髓內(nèi)釘（PFNA）對(duì)老年股骨粗隆間骨折患者療效的影響。方法：選取2016年5月-2017年6月本院收治的老年股骨粗隆間骨折患者112例。按照隨機(jī)數(shù)字表法將其分為觀察組和對(duì)照組，每組各56例。觀察組采用PFNA固定治療，對(duì)照組采用股骨髁上骨牽引治療。比較兩組術(shù)后疼痛程度（VAS評(píng)分）、手術(shù)時(shí)間、術(shù)中出血量、下地部分負(fù)重時(shí)間、治療效果及術(shù)后并發(fā)癥發(fā)生情況。結(jié)果：術(shù)后1周，兩組VAS評(píng)分比較差異無(wú)統(tǒng)計(jì)學(xué)意義（P>0.05）；術(shù)后2～4周，觀察組VAS評(píng)分均低于對(duì)照組，比較差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）。觀察組患者手術(shù)時(shí)間、術(shù)中出血量及下地部分負(fù)重時(shí)間均少于對(duì)照組，比較差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）；觀察組總優(yōu)良率為92.86%，高于對(duì)照組的73.21%（ 字2=7.669，P=0.006）；觀察組并發(fā)癥發(fā)生率為8.93%，低于對(duì)照組的39.29%（ 字2=14.104，P=0.000）。結(jié)論：PFNA治療老年股骨粗隆間骨折，可降低患者疼痛感及并發(fā)癥發(fā)生率，提高療效，值得推廣。

　　【Abstract】 Objective：To explore the effect of minimally invasive proximal femoral nail antirotation（PFNA） on elderly patients with intertrochanteric fractures of femur.Method：A total of 112 elderly patients with intertrochanteric fracture of femur from May 2016 to June 2017 in our hospital were selected.According to the random number table method，they were divided into observation group and control group，56 cases in each group.The observation group was treated with PFNA fixation，and control group was treated with femoral supracondylar bone traction.The postoperative pain degree（VAS score），operative time，intraoperative blood loss，partial weight loading time，therapeutic effect and postoperative complications of two groups were compared.Result：After operation 1 week，the VAS scores of two groups were compared，the difference was not statistically significant（P>0.05），after operation 2-4 weeks， the VAS scores of observation group were lower than those of control group，the differences were statistically significant（P<0.05）.The operative time，intraoperative blood loss，partial weight loading time in observation group were less than those of control group，the differences were statistically significant（P<0.05）.The total excellent rate of observation group was 92.86%，which was higher than 73.21% of control group（ 字2=7.669，P=0.006）.The incidence of complications in observation group was 8.93%，which was lower than 39.29% of control group（ 字2=14.104，P=0.000）.Conclusion：The treatment of intertrochanteric fracture of femur by PFNA can reduce the incidence of pain and complications，improve the curative effect，which is worth popularizing.
