[摘要]目的 探討全科團(tuán)隊(duì)干預(yù)模式對(duì)社區(qū)糖尿病患者管理的影響。方法 選取2015年3月～2016年12月我中心收治的94例糖尿病患者作為研究對(duì)象，以隨機(jī)數(shù)表抽取法將其分為對(duì)照組和觀察組，每組各47例。對(duì)照組實(shí)施傳統(tǒng)糖尿病社區(qū)管理模式，觀察組給予全科團(tuán)隊(duì)干預(yù)模式，觀察對(duì)比兩組患者的生活質(zhì)量和糖尿病控制狀況。結(jié)果 觀察組患者QOL-100量表中心理功能、生理功能、精神寄托、社會(huì)功能、周圍環(huán)境5項(xiàng)得分均高于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；觀察組患者糖尿病及并發(fā)癥認(rèn)知、生活習(xí)慣、治療情況、生存技能、治療目標(biāo)5個(gè)模塊得分均高于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 全科團(tuán)隊(duì)干預(yù)模式在社區(qū)糖尿病患者管理中起到重要的作用，能夠有效改善患者的生活質(zhì)量和身體機(jī)能，維持患者的病情穩(wěn)定，值得廣泛推廣。

　　[Abstract]Objective To explore the influence of general practitioner team intervention on the management of diabetic patients in the community.Methods From March 2015 to December 2016，94 patients with diabetes in our center were selected as the research objects，and divided into control group and observation group by random number tables method，47 cases in each group.In the control group，the traditional diabetes community management mode was used，while in the observation group，general practitioner team intervention was provided.The quality of life and diabetic control in the two groups were observed and compared.Results In the observation group，the scores of psychological function，physiologicalfunction，spiritualballast，socialfunction，and surroundings in QOL-100 scale were all higher than those in the control group，the difference was statistically significant （P<0.05）.The scores of cognition on diabetes and its complications，livinghabit，treatment，survial skills，therapeutic goal，and knowledge structure in the observation group were higher than those in the control group，the difference was statistically significant （P<0.05）.Conclusion General practitioner team intervention mode plays an important role in the management of community diabetes patients，can effectively improve the quality of life and physical function of patients，maintain the stability of patients，which is worthy of wide promotion.

　　糖尿病是一種代謝性疾病，隨著病情的發(fā)展會(huì)誘發(fā)神經(jīng)、心腦和血管等多種組織器官病變，導(dǎo)致其功能障礙，嚴(yán)重威脅患者的身體健康和生命安全[1]。隨著人們生活水平的提高和飲食結(jié)構(gòu)的改變，糖尿病發(fā)病率呈現(xiàn)逐年上升的趨勢(shì)，糖尿病防治工作日漸成為社區(qū)慢性病管理工作的重點(diǎn)，而傳統(tǒng)糖尿病護(hù)理模式存在明顯的局限性，工作人員必須結(jié)合病情發(fā)展的實(shí)際情況，在具體工作中創(chuàng)建社區(qū)糖尿病綜合管理模式，其中全科團(tuán)隊(duì)干預(yù)模式優(yōu)勢(shì)顯著，能夠?yàn)樘悄虿』颊咛峁┤�面專業(yè)的護(hù)理服務(wù)，被廣泛應(yīng)用于社區(qū)糖尿病管理工作中[2]。本研究基于上述背景，探討了全科團(tuán)隊(duì)干預(yù)模式對(duì)社區(qū)糖尿病患者管理的影響，現(xiàn)報(bào)道如下。
