摘   要：目的  探討脂蛋白相關(guān)性磷脂酶A2（Lp-PLA2）、血小板相關(guān)因子與2型糖尿病合并高血壓病患者的相關(guān)性。方法  連續(xù)入選我院2型糖尿病患者共102例，分為2型糖尿病合并高血壓病組（n=50）、單純2型糖尿病組（n=52），以及同期住院的單純高血壓病組（n=30）、體檢健康對(duì)照組（n=30）。記錄四組患者一般資料、生化指標(biāo)及血小板相關(guān)因子--血小板計(jì)數(shù)（PLT）、血小板分布寬度（PDW）、平均血小板容積（MPV）及大血小板比率（P-LCR）水平，并測(cè)定Lp-PLA2水平。結(jié)果  四組間性別、年齡、ALT、AST、Scr、BUN、TCH比較，差異無統(tǒng)計(jì)學(xué)意義（P>0.05）。2型糖尿病合并高血壓病患者的Lp-PLA2及PDW、MPV、P-LCR水平較單純2型糖尿病患者、單純高血壓病組及健康對(duì)照組升高，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）;PLT較單純2型糖尿病組、單純高血壓病組及健康對(duì)照組降低，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。Lp-PLA2、PDW與收縮壓（SBP）成正相關(guān)（P<0.05），Lp-PLA2、PDW、MPW、P-LCT與糖化血紅蛋白（HbA1c）成正相關(guān)（P<0.05）。結(jié)論  Lp-PLA2、血小板相關(guān)因子可評(píng)估2型糖尿病患者血管內(nèi)皮功能狀態(tài)，合并高血壓病的2型糖尿病患者血管內(nèi)皮功能障礙更加嚴(yán)重，且該嚴(yán)重程度與血壓、血糖控制不佳的程度有關(guān)。

　　Abstract：Objective  To investigate the relationship between lipoprotein associated phospholipase A2（Lp-PLA2），platelet related factors and type 2 diabetes mellitus complicated with hypertension.Methods  102 patients with type 2 diabetes mellitus were eolled in our hospital.The patients were divided into two groups：type 2 diabetes mellitus complicated with hypertension group（n=50），type 2 diabetes group（n=52），hypertension group（n=30）and healthy control group（n=30）.The general data， biochemical parameters and platelet related factor-platelet count（PLT），platelet distribution width（PDW），mean platelet volume（MPV）and large platelet ratio（P-LCR）were recorded in the four groups，and the level of Lp-PLA2 was measured.Results  There was no significant difference in the age，sex，ALT，AST，Scr，BUN，TCH among the four groups（P>0.05）.Type 2 diabetes mellitus and hypertension in patients with Lp-PLA2 and PDW， MPV and P-LCR levels higher than patients with type 2 diabetes mellitus，hypertension group and healthy control group increased，the difference was statistically significant（P<0.05）;PLT compared with type 2 diabetes group and simple hypertension group and healthy control group decreased，with statistical significance the difference（P<0.05）.Lp-PLA2 and PDW were positively correlated with systolic blood pressure（SBP）（P<0.05），and Lp-PLA2，PDW，MPW and P-LCT were positively correlated with glycosylated hemoglobin（HbA1c） （P<0.05）.Conclusion  Lp-PLA2 and platelet related factors can evaluate endothelial function in patients with type 2 diabetes mellitus， and endothelial dysfunction is more severe in type 2 diabetic patients with hypertension，and the severity is related to the degree of blood pressure and blood sugar control.
