【摘要】 目的：探討腔鏡膨脹液在頸部切口甲狀腺手術(shù)中應(yīng)用的可行性。方法：選取2016年9-11月本院甲狀腺外科擬行甲狀腺手術(shù)患者，按照隨機數(shù)字表法分為傳統(tǒng)組（A組）與改良組（B組），B組應(yīng)用腔鏡膨脹液。比較兩組分離皮瓣時間、術(shù)區(qū)時間、手術(shù)總時間、分離皮瓣出血量、術(shù)區(qū)出血量、術(shù)中總出血量、皮瓣分離前后血壓、心率及血氧飽和度變化，術(shù)區(qū)引流量、皮下引流量、術(shù)后引流總量、術(shù)后痛覺評分、術(shù)后住院天數(shù)，統(tǒng)計術(shù)后出血、切口及術(shù)區(qū)感染例數(shù)。結(jié)果：兩組均無術(shù)后出血、切口及術(shù)區(qū)感染等并發(fā)癥。B組分離皮瓣時間、手術(shù)總時間、分離皮瓣出血量、術(shù)中總出血量、皮下引流量明顯優(yōu)于A組，B組術(shù)后痛覺評分明顯小于A組，比較差異均有統(tǒng)計學(xué)意義（P<0.05）。術(shù)中總出血量與皮瓣出血量、術(shù)區(qū)出血量之間存在正相關(guān)性，手術(shù)總時間與分離皮瓣時間、術(shù)區(qū)時間之間存在正相關(guān)性，而術(shù)后引流總量只與術(shù)區(qū)引流量相關(guān)。結(jié)論：腔鏡膨脹液在頸部切口甲狀腺手術(shù)中應(yīng)用可有效減少建立皮瓣分離時間、術(shù)區(qū)時間、術(shù)區(qū)出血量，并可減輕術(shù)后疼痛，增加術(shù)后舒適度，安全有效。

　　【Abstract】 Objective：To explore the feasibility of the application of endoscopic inflation fluid in traditional thyroidectomy.Method：Patients undergoing thyroidectomy from September to November 2016 in our hospital were randomly divided into traditional group and modified group which was treated by endoscopic inflation fluid.The separated flaps time，intraoperative time，totaloperation time，separated flapsblood loss，intraoperative blood loss，totaloperationblood loss，the changes of blood pressure during separated flaps，heart rate and Oxygen saturation（SpO2），intraoperativedrainage，subcutaneous drainage，post-operation drainage，post-operative pain score，post-operative hospital stay，and the number of post-operative blood loss and incision infection were respectively recorded and analyzed.Result：There were no complications of postoperative hemorrhage and incision infection in two groups.The separated flaps time，total operative time，separated flaps blood loss，total operative blood loss，subcutaneous drainage，post-operative pain score of modified group were less than those of traditional group，the differences were statistically significant（P<0.05）.The separated flaps time，intraoperative time，total operation time were positively correlated with each other.These parated flaps blood loss，intraoperative blood loss，total operation blood loss were positively correlated with each other.The subcutaneous drainage and post-operation drainagewerepositively correlated with each other.Conclusion：Endoscopic inflation fluid was applied to traditional thyroidectomy，which might apparently reduce the separated flaps time，total operative time，intraoperative blood loss and alleviate post-operative pain.Therefore，the application of endoscopic inflation fluid is a safe and effective method for traditional thyroidectomy.
