【摘要】目的 觀察并評(píng)價(jià)中藥灌腸聯(lián)合高強(qiáng)度聚焦超聲（HIFU）治療子宮肌瘤療效。方法 選擇2015年11月～2017年5月期間在我院接受治療的60例宮肌瘤患者，按照接診日期單雙數(shù)分為對(duì)照組與觀察組各30例，對(duì)照組采用HIFU治療，觀察組在HIFU基礎(chǔ)上應(yīng)用中藥灌腸療法，對(duì)兩組患者臨床治療效果進(jìn)行統(tǒng)計(jì)與處理。結(jié)果 觀察組有效率96.7%，顯著高于對(duì)照組73.3%，組間差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；治療前兩組患者肌瘤直徑與體積指標(biāo)比較差異無統(tǒng)計(jì)學(xué)意義（P>0.05）；治療后均有所減縮，觀察組患者上述指標(biāo)改善效果顯著好于對(duì)照組，組間差異顯著（P<0.05）；兩組患者治療與隨訪期間均沒出現(xiàn)嚴(yán)重并發(fā)癥。結(jié)論 中藥灌腸聯(lián)合高強(qiáng)度聚焦超聲治療子宮肌瘤效果優(yōu)良，具有一定推廣價(jià)值。

　　【Abstract】Objective To observe and evaluate the efficacy of traditional Chinese medicine enema combined with high intensity focused ultrasound （HIFU） in the treatment of hysteromyoma.Methods 60 cases of uterine fibroids patients choose November 2015 ～2017 year in May in our hospital，according to the date of receiving single and double divided into control group and observation group with 30 cases in each group，the control group was treated with HIFU，the observation group in the HIFU on the basis of the application of traditional Chinese medicine enema， statistics and processing of two groups of patients with clinical treatment effect.Results The effective rate of observation group was 96.7%， significantly higher than 73.3% in the control group， there was significant difference between the groups （P<0.05）； no statistical significance in two groups of patients before treatment of myoma diameter and volume index difference （P>0.05）；after treatment were dropped，the patients in the observation group improved significantly better than the control index group， significant differences between the groups （P<0.05）；two groups of patients during treatment and follow-up were not serious complications.Conclusion The effect of traditional Chinese medicine enema combined with high intensity focused ultrasound in the treatment of hysteromyoma has a good effect， and has a certain promotion value.

　　子宮肌瘤是臨床中一種常見良性腫瘤，育齡女性是本病好發(fā)群體，絕經(jīng)后肌瘤直徑與體積均有所減縮。有統(tǒng)計(jì)資料顯示，子宮肌瘤在育齡女性中發(fā)生率為19.1%，最近幾年中有上漲趨勢(shì)，對(duì)患者生活質(zhì)量造成顯著影響，積極研討子宮肌瘤有效治療方案是現(xiàn)代醫(yī)學(xué)重要課題之一[1]。
　　1 資料與方法
　　1.1 一般資料
　　取2015年11月～2017年8月期間在我院接受治療的60例子宮肌瘤患者的臨床資料為研究對(duì)象，均為育齡女性，經(jīng)影像學(xué)檢查并由我院婦科專家診斷為子宮肌瘤，符合中華醫(yī)學(xué)會(huì)婦科分會(huì)編制的《子宮肌瘤診斷標(biāo)準(zhǔn)》。兩組患者一般資料經(jīng)統(tǒng)計(jì)學(xué)分析，組間差異無統(tǒng)計(jì)學(xué)意義（P>0.05）。
　　1.2 方法
　　1.2.1 對(duì)照組
　　采用HIFU單一治療方法。設(shè)備：北京源德醫(yī)療設(shè)備生產(chǎn)的FEP-BY02型高能聚焦超聲腫瘤治療機(jī)，治療全程由計(jì)算機(jī)自動(dòng)控制和手動(dòng)控制進(jìn)行。每次治療期間指導(dǎo)患者服用芬必得0.2 g，2次/d，持續(xù)給藥至療程結(jié)束。
　　1.2.2 觀察組
　　在對(duì)照組治療基礎(chǔ)上，在治療后第2d施以中藥消瘤灌腸方保留灌腸，消瘤灌腸方劑：毛冬青、忍冬藤、大黃、夏枯草、皂角刺、路路通、三棱、莪術(shù)、丹參，結(jié)合我院制劑標(biāo)準(zhǔn)濃煎過濾，100 mL/袋 。囑患者需保留灌腸時(shí)間>4 h，1次/d，連續(xù)給藥7 d。治療后半年中對(duì)患者進(jìn)行隨訪。
