[摘要]目的 探討社區(qū)中醫(yī)護理對老年類風(fēng)濕關(guān)節(jié)炎患者生活質(zhì)量的影響。方法 收集2014年1月～2016年1月我社區(qū)老年風(fēng)濕關(guān)節(jié)炎患者92例臨床資料，依據(jù)患者的意愿分為對照組和實驗組，每組46例，對照組患者采用臨床常規(guī)方法治療和護理，實驗組患者在對照組患者治療和護理的基礎(chǔ)上實施社區(qū)中醫(yī)護理，用SF-36生存質(zhì)量量表對患者的生存質(zhì)量進行評估并比較兩組患者的康復(fù)效果。結(jié)果 社區(qū)中醫(yī)護理后實驗組的生存質(zhì)量評分高于對照組，差異有統(tǒng)計學(xué)意義（t=8.694，P=0.000）；實驗組總有效率97.40%，顯著優(yōu)于對照組的77.10%，差異有統(tǒng)計學(xué)意義（P<0.01）。結(jié)論 開展社區(qū)中醫(yī)護理對老年人類風(fēng)濕關(guān)節(jié)炎生存質(zhì)量有著積極的作用，值得臨床上推廣應(yīng)用。

　　Influence of community traditional Chinese medicine nursing on quality of life in elderly patients with rheumatoid arthritis

　　[Abstract]Objective To explore the influence of community traditional Chinese medicine （TCM） nursing on the quality of life （QOL） in elderly patients with rheumatoid arthritis.Methods The clinical data of 92 senile patients with rheumatoid arthritis in our community from January 2014 to January 2016 were collected.According to patient′s will，they were evenly divided into the experimental group and the control group，with 46 cases in each group，in the control group，the clinical conventional routine method and nursing was used，in the experimental group，community TCM nursing was applied on the basis of the control group.The patient′s QOL was assessed by SF-36 scale，and the rehabilitation effect was compared between the two groups.Results The quality of life score of the experimental group was higher than that of the control group， the difference was statistically significant （t=8.694，P=0.000）.The total effectiveness rate in the experimental group was 97.40%，which was significantly better than that of the control group accounting for 77.10% with a statistical difference （P<0.01）.Conclusion Implementation of the community TCM nursing has a positive effect on QOL in the elderly patients with rheumatoid arthritis，and it is worthy of clinical application.

　　類風(fēng)濕關(guān)節(jié)炎屬于一種以侵蝕性、對稱性為基本特征表現(xiàn)的關(guān)節(jié)炎癥反應(yīng)類疾病，同屬也是慢性、全身性自身免疫性疾病的一種。本病呈全球性分布，是目前臨床乃至全社會公認的一種導(dǎo)致人類勞動力喪失和致殘的一個非常主要的原因。根據(jù)相關(guān)領(lǐng)域所進行的研究資料顯示[1]，該病的發(fā)病率為0.32%～0.36%，由于類風(fēng)濕關(guān)節(jié)炎需終身治療，治療時間長，預(yù)后效果往往不佳[2-3]。該疾病所具有的高致殘性，可以使廣大患者在患病時候的短時間內(nèi)迅速喪失勞動能力，生活不能自理，使情緒出現(xiàn)異常，對生活質(zhì)量造成嚴重影響[4]。故而在進行治療的過程后，需給予適當?shù)纳鐓^(qū)護理，以改善患者的情緒、生活質(zhì)量。有研究顯示，社區(qū)中醫(yī)護理應(yīng)用于類風(fēng)濕關(guān)節(jié)炎中，能夠顯著改善患者的情緒、康復(fù)效果及生活質(zhì)量[5]�，F(xiàn)筆者為探討社區(qū)中醫(yī)護理對類風(fēng)濕關(guān)節(jié)炎患者預(yù)后及生活質(zhì)量的影響，將類風(fēng)濕性關(guān)節(jié)炎患者分為兩組，分別應(yīng)用常規(guī)護理、社區(qū)中醫(yī)護理，比較其康復(fù)效果及生活質(zhì)量，現(xiàn)報道如下。
