糖尿病周圍神經(jīng)損傷上肢受累分布特點的研究

[摘要]目的 探討糖尿病周圍神經(jīng)損傷上肢受累的分布特點。方法 選取2016年3月~2017年5月我院收治的300例糖尿病患者進行資料分析,將150例糖尿病周圍神經(jīng)病患者作為觀察組,150例無周圍神經(jīng)疾病的糖尿病患者作為對照組,對所有患者進行肌電/誘發(fā)電位儀檢測上肢正中神經(jīng)、橈神經(jīng)以及尺神經(jīng)的感覺神經(jīng)傳導(dǎo)速度(SCV)、感覺神經(jīng)傳導(dǎo)波幅(SNAP)、運動神經(jīng)傳導(dǎo)速度(MCV)、符合肌肉動作電位波幅(CAMP)。比較兩組患者的神經(jīng)傳導(dǎo)指標(biāo)。結(jié)果 觀察組患者的正中神經(jīng)、橈神經(jīng)、尺神經(jīng)SCV、MCV均低于對照組,差異有統(tǒng)計學(xué)意義(P<0.05)。不同神經(jīng)比較,正中神經(jīng)、運動神經(jīng)與感覺神經(jīng)異常率高于其他神經(jīng)(P<0.05);尺神經(jīng)與橈神經(jīng)的感覺神經(jīng)異常率以及運動神經(jīng)異常率比較,差異無統(tǒng)計學(xué)意義(P>0.05)。同一感覺神經(jīng)與運動神經(jīng)比較,正中神經(jīng)感覺神經(jīng)異常率高于運動神經(jīng)異常率,差異有統(tǒng)計學(xué)意義(P<0.05);尺神經(jīng)與橈神經(jīng)的感覺神經(jīng)以及運動神經(jīng)異常率比較,差異無統(tǒng)計學(xué)意義(P>0.05)。結(jié)論 糖尿病周圍神經(jīng)病(DPN)患者的神經(jīng)傳導(dǎo)速度明顯低于無周圍神經(jīng)疾病的傳導(dǎo)速度,較無周圍神經(jīng)疾病的糖尿病患者的上肢神經(jīng)以及正中神經(jīng)受累也更為嚴(yán)重。

Study on distribution characteristics of upper extremities affected by diabetic peripheral nerve injury

[Abstract]Objective To investigate the distribution characteristics of upper extremities affected by diabetic peripheral nerve injury.Methods 300 patients with diabetes treated in our hospital from March 2016 to May 2017 were selected and their data were analyzed.150 patients with diabetic peripheral neuropathy were selected as the observation group.150 diabetic patients without peripheral neuropathy were selected as the control group.All patients were examined by electromyography/evoked potentiometry for the detection of sensory nerve conduction velocity (SCV),sensory nerve conduction amplitude (SNAP),motor nerve conduction velocity (MCV),compound muscle action potential amplitude (CAMP) in the median nerve,radial nerve and ulnar nerve of upper extremities.The nerve conduction parameters were compared between the two groups.Results The SCV and MCV of median nerve,radial nerve and ulnar nerve in the observation group were lower than those in the control group,the difference was statistically significant (P<0.05).Different nerves were compared,the abnormal rates of median nerve,motor nerve and sensory nerve were higher than those of other nerves (P<0.05).There was no significant difference in the abnormal rates of sensory nerve and motor nerve between the ulnar nerve and radial nerve (P>0.05).The sensory nerve,motor nerve,and median nerve were compared,and the abnormal rate of sensory nerve was higher than that of motor nerve,the difference was statistically significant (P<0.05).There was no significant difference in the abnormal rate of sensory nerve and motor nerve between the ulnar nerve and radial nerve (P>0.05).Conclusion The nerve conduction in the patients with diabetic peripheral neuropathy is significantly lower than that in the diabetic patients without peripheral neuropathy,and compared with the diabetic patients without peripheral neuropathy,the affected nerve of upper extremities and the affected median nerve are also the more severe.
