[摘要]目的 探討咽鼓管球囊擴(kuò)張術(shù)（ETBD）治療慢性分泌性中耳炎的臨床效果。方法 選擇普寧市人民醫(yī)院耳鼻喉科2015年1月～2016年2月收治的慢性分泌性中耳炎患者57例（61耳），按入院順序隨機(jī)分為觀察組29例（31耳）、對(duì)照組28例（30耳）。觀察組患者給予ETBD，對(duì)照組給予鼓膜置管術(shù)。比較兩組患者的咽鼓管功能障礙（ETD）情況、純音聽(tīng)閾值（PTA），治療效果。結(jié)果 術(shù)后6、12、18個(gè)月，觀察組患者的ETDQ-7評(píng)分顯著低于對(duì)照組（P<0.05）。術(shù)后12個(gè)月，觀察組患者的PTA顯著低于對(duì)照組（P<0.05）。術(shù)后12、18個(gè)月，觀察組的有效率（83.9%、93.5%）顯著高于對(duì)照組（56.7%、43.3%）（P<0.05）。結(jié)論 ETBD治療慢性分泌性中耳炎可顯著緩解患者的臨床癥狀，提高臨床效果。

　　Effect analysis of the eustachian tube balloon dilation treating chronic secretory otitis media

　　[Abstract]Objective To discuss the clinical effect of the eustachian tube balloon dilation （ETBD） treating chronic secretory otitis media.Methods 57 patients （61 ears） with chronic secretory otitis media treated in People′s Hospital of Puning City from January 2015 to February 2016 were selected and they were randomly divided into the observation group （29 patients，31 ears） and control group （28 patients，30 ears） according to hospital sequence.Patients in the observation group were treated with ETBD，while patients in the control group were treated with grommet insertion.Eustachian tube dysfunction （ETD），pure toneaudiometry （PTA） and treatment effect were compared between the two groups.Results ETDQ-7 score in patients of the observation group was significantly lower than those of the control group after 6，12 and 18 months operation （P<0.05）.PTA in patients of the observation group was significantly lower than that of the control group after 12 months operation （P<0.05）.The effective rate of the observation group （83.9%，93.5%） was significantly higher than that of the control group （56.7%，43.3%） at after 12，18 months operation （P<0.05）.Conclusion ETBD treating chronic secretory otitis media can significantly relieve the clinical symptoms of the patients and improve the clinical effect.

　　咽鼓管功能障礙（eustachian tube dysfuntion，ETD）主要指因多種因素導(dǎo)致咽鼓管發(fā)生通氣功能不良，使中耳氣壓失去平衡的功能障礙。典型ETD可表現(xiàn)為隨外界氣壓的變化，耳部出現(xiàn)悶塞感、完成Valsalva動(dòng)作困難等。成人ETD發(fā)生率為0.9%～5%，<10歲的兒童中約40%至少發(fā)生過(guò)1次ETD經(jīng)歷，因此，ETD是臨床耳鼻喉科常見(jiàn)疾病[1]。慢性分泌性中耳炎主要臨床表現(xiàn)為耳部悶脹感、聽(tīng)力不同程度下降等，該病發(fā)病機(jī)制至今上不明確，病因復(fù)雜，多數(shù)學(xué)者認(rèn)為是由ETD、咽鼓管阻塞導(dǎo)致。手術(shù)是治療慢性分泌性中耳炎的重要治療方法，咽鼓管球囊擴(kuò)張術(shù)（eustachian tube balloon dilation，ETBD）因其具有操作簡(jiǎn)單、創(chuàng)傷性小、安全性高、有效率高等優(yōu)點(diǎn)，備受臨床醫(yī)生的關(guān)注[2-3]。本研究采用ETBD與鼓膜置管術(shù)治療慢性分泌性中耳炎，并比較兩種治療方法的應(yīng)用價(jià)值，現(xiàn)報(bào)道如下。
