摘 要 目的：為制定舒緩療護收費標準提供依據(jù)。方法：選取2016年上海市某老年護理院舒緩療護病區(qū)晚期惡性腫瘤出院患者42例，與同時期一般老年護理病區(qū)出院患者123例，對兩組患者的住院費用進行比較分析。結(jié)果：舒緩療護病區(qū)患者的人均住院天數(shù)明顯低于同期一般老年護理病區(qū)，而舒緩療護病區(qū)患者的日均費用、日均藥費明顯高于同期一般老年護理病區(qū)，其差異均有統(tǒng)計學意義（P<0.05）。結(jié)論：舒緩療護病區(qū)晚期惡性腫瘤臨終患者與一般老年護理患者住院費用有差異，應(yīng)將相應(yīng)的服務(wù)內(nèi)容列入到醫(yī)療保險收費項目中。

　　Analysis and study of palliative nursing and hospitalization expenses of elderly patients nursed in an elderly nursing home

　　ABSTRACT Objective： To provide the basis for setting up the charge standard of palliative care. Methods： In 2016， 42 patients with advanced malignant tumors discharged from an elderly nursing home in Shanghai and 123 cases discharged from the general nursing wards at the same time were selected， and the hospitalization expenses of the two groups were compared and analyzed. Results： The per capita hospitalization days of patients in the palliative care words were significantly lower than those in the general elderly nursing wards but the daily cost and daily average charge of patients in the palliative care words were significantly higher than those in the elderly nursing wards and the differences were statistically significant（P<0.05）. Conclusion： The cost of hospitalization of terminal patients with advanced malignant tumors in the palliative nursing wards is different from that of the general elderly patients nursed， and the corresponding service content should be included in the medical insurance fee projects.

　　在上海市人口日益老齡化、癌癥成為人口死亡重要原因的前提下，臨終患者對于舒緩療護的需求越來越迫切。世界衛(wèi)生組織（WHO）指出，舒緩療護是一系列照護方法，它通過運用早期確認、準確評估和完善治療身體病痛和心理及精神疾患來干預并緩解患者痛苦，以此提高罹患威脅生命疾病的患者及其家屬的生活質(zhì)量[1]。
　　上海市黃浦區(qū)順昌醫(yī)院被列入2014年上海市政府實事工程項目，開展第二批舒緩療護病區(qū)建設(shè)。為分析舒緩療護病區(qū)晚期惡性腫瘤患者與一般老年護理病區(qū)患者住院費用的差別，以上海市黃浦區(qū)順昌醫(yī)院2016年舒緩療護病區(qū)晚期惡性腫瘤出院患者為研究組，與同期一般老年護理二病區(qū)出院患者的住院費用情況進行比較分析，為政府制定舒緩療護服務(wù)內(nèi)容與收費標準提供依據(jù)。
　　1 資料與方法
　　1.1 一般資料
　　選取2016年上海市某老年護理院舒緩療護病區(qū)晚期惡性腫瘤出院患者42例作為研究組，選取同時期一般老年護理病區(qū)出院患者123例作為對照組，對兩組患者的住院費用進行比較分析。
　　研究組患者42例，其中男性9例，女性33例；年齡52～98歲，平均年齡（81±10）歲；患者罹患消化道腫瘤（食管癌、胃癌、腸癌、肝癌、胰腺癌）13例，肺癌8例，乳腺癌4例，顱內(nèi)腫瘤10例，其他7例；出院時好轉(zhuǎn)10例，未愈9例，死亡23例。
　　對照組患者123例，其中男性43例，女性80例；年齡58～93歲，平均年齡（83±8）歲；患者罹患腦梗死31例，癡呆77例，冠心病9例，其他6例；出院時好轉(zhuǎn)106例，死亡17例。
　　1.2 方法
　　從醫(yī)院計算機系統(tǒng)中導出2016年舒緩療護病區(qū)出院的晚期惡性腫瘤患者與同期一般老年護理二病區(qū)出院患者的基本資料及住院期間所有費用數(shù)據(jù)，包括患者疾病種類、住院天數(shù)、日均費用及日均費用的具體組成情況。
