【摘要】 目的：探討經(jīng)支氣管鏡對(duì)局部病灶行支氣管肺泡灌洗加全身抗感染治療重癥肺部感染患者的療效。方法：選取本院2015年7月-2017年7月收治的48例重癥肺部感染患者，根據(jù)治療方式的不同分為對(duì)照組和觀察組，每組各24例。對(duì)照組采取全身抗感染治療，觀察組在此基礎(chǔ)上，采取經(jīng)支氣管鏡對(duì)局部病灶行支氣管肺泡灌洗治療。觀察對(duì)比兩組治療前后血?dú)夥治鲋笜?biāo)（SpO2、PaCO2、PaO2）變化、住院時(shí)間、癥狀消失時(shí)間以及臨床療效。結(jié)果：治療后觀察組SpO2、PaCO2、PaO2等指標(biāo)與對(duì)照組相比改善顯著，比較差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）。觀察組住院時(shí)間、癥狀消失時(shí)間等明顯短于對(duì)照組，比較差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）。觀察組治療有效率為95.83%（23/24）明顯高于對(duì)照組的79.17%（19/24），差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論：針對(duì)重癥肺部感染患者采取經(jīng)支氣管鏡對(duì)局部病灶行支氣管肺泡灌洗加全身抗感染治療的效果非常顯著，既能顯著改善患者各種癥狀，又能縮短其住院時(shí)間，加速其病情恢復(fù)，因此值得臨床應(yīng)用推廣。

　　【Abstract】 Objective：To evaluate the efficacy of bronchoscopies for the treatment of patients with severe pulmonary infection by bronchoalveolar lavage plus systemic anti-infection.Method：48 patients with severe pulmonary infection admitted from July 2015 to July 2017 in our hospital were divided into control group and observation group according to the different treatment methods，24 cases in each group.The control group was treated with systemic anti-infective therapy，on this base，the observation group was treated with bronchoalveolar lavage through bronchoscopy.The changes of blood gas analysis indexes （SpO2，PaCO2，PaO2），length of hospital stay，the time of symptom disappearance and the clinical efficacy of two groups were observed and compared before and after treatment.Result：After treatment， SpO2， PaCO2， PaO2 and others in the observation group improved significantly compared with the control group，the difference was statistically significience（P<0.05）.The observation group hospitalization time and other symptoms disappeared significantly shorter compared with the control group，the differences were statistically significience（P<0.05）.The effective rate in observation group was 95.83% significantly higher than 79.17% in the control group，the difference was statistically significant（P<0.05）.Conclusion：The treatment of severe pulmonary infection by bronchoscopy for local lesions with bronchoalveolar lavage plus systemic anti-infective therapy is very significant，both significantly improve the symptoms of patients，but also shorten their hospital stay and accelerate the recovery of their condition，it is worth to promote clinical application.
