[摘要]目的 觀察重癥肺炎患者血清中活化蛋白C（APC）的表達水平，評判其在評估重癥肺炎患者預(yù)后評估中的價值。方法 選擇2013年10月～2016年7月我科診治重癥肺炎患者54例，按預(yù)后分為存活組（30例）和死亡組（24例）。隨機選取同期我院的非重癥肺炎患者30例作為非重癥肺炎組，選取同期在我院查體的健康人30例作為健康組，采用酶聯(lián)免疫吸附法檢測各組血清中的APC水平。結(jié)果 肺炎組患者與健康人比較，肺炎組的APC水平為（330.81±72.80）ng/ml，低于健康組[（436.57±62.69）ng/ml]，差異有統(tǒng)計學意義（P<0.05）；重癥肺炎組的APC水平為（296.60±58.26）ng/ml，低于非重癥肺炎組[（392.38±53.52）ng/ml]，差異有統(tǒng)計學意義（P<0.05）；死亡組的APC水平為（263.89±53.53）ng/ml，低于存活組[（322.75±48.34）ng/ml]，差異有統(tǒng)計學意義（P<0.05）。結(jié)論 血清中APC水平與肺炎相關(guān)，APC水平反應(yīng)患者疾病嚴重程度，且與重癥肺炎預(yù)后相關(guān)，APC水平越低，患者預(yù)后越差。

　　The application value of APC level for the prognosis assessment of patients with severe pneumonia

　　[Abstract]Objective To observe the level of serum activated protein C（APC） in patients with severe pneumonia and evaluate its value in evaluating the prognosis of patients with severe pneumonia.Methods A total of 54 patients with severe pneumonia were selected from October 2013 to July 2016，the prognosis was divided into survival group （n=30） and death group （n=24）.30 patients with non-severe pneumonia were selected randomly in the same period，as a non-severe pneumonia group.A healthy group of 30 healthy people were selected in our hospital during the same period.Using enzyme-linked immunosorbent method to detect each group APC level in the serum.Results The APC level of the pneumonia group was （330.81±72.80） ng/ml，which was lower than that the healthy group [（436.57±62.69） ng/ml]，and the difference was statistically significant（P<0.05）.The APC level of the severe pneumonia group was （296.60±58.26） ng/ml，which was lower than that the non-severe pneumonia group [（392.38±552） ng/ml]，and the difference was statistically significant （P<0.05）.The APC level of the death group was （263.89±53.53） ng/ml，which was lower than that the survival group [（322.75±48.34） ng/ml]， and the difference was statistically significant （P<0.05）.Conclusion Level of APC in serum is associated with pneumonia，and the level of APC reflects the severity of the disease and is related to the prognosis of severe pneumonia.The lower the APC level，the worse the prognosis of the patients.
