超聲心動圖對肥厚型心肌病的診斷價值

[摘要] 目的 探討超聲心動圖對肥厚型心肌病的診斷價值。 方法 選擇我院2012年3月~2017年9月確診的76例肥厚型心肌病及76例健康人群分別作為研究組和對照組,對其超聲心動圖特點進行回顧性分析。研究組患者進行24 h動態(tài)心電圖檢查,分析比較梗阻性肥厚型心肌病與非梗阻性肥厚型心肌病心律失常情況。 結(jié)果 研究組與對照組在左室舒張末徑(LVIDd)、左室收縮末徑(LVISd)方面比較差異無統(tǒng)計學(xué)意義(P>0.05),在室間隔厚度、左室后壁厚度、左房前后徑方面比較差異有統(tǒng)計學(xué)意義(P<0.05);梗阻性和非梗阻性肥厚型心肌病在心律失常方面差異有統(tǒng)計學(xué)意義(P<0.05),梗阻性較非梗阻性肥厚型心肌病更容易出現(xiàn)心律失常,以房性、室性心律失常常見。 結(jié)論 超聲心動圖操作簡便且無創(chuàng)傷,可以快速地對肥厚型心肌病做出診斷及分型,為臨床的診治提供快速而準確的依據(jù)。

The value of echocardiography in diagnosis of hypertrophic cardiomyopathy

[Abstract] Objective To investigate the diagnostic value of echocardiography in hypertrophic cardiomyopathy. Methods 76 cases of hypertrophic cardiomyopathy and 76 cases of healthy people diagnosed in our hospital from March 2012 to September 2017 were selected as the study group and the control group respectively. The characteristics of echocardiography were analyzed retrospectively. The patients in the study group were examined by 24 hours ambulatory electrocardiogram, and the arrhythmias of obstructive hypertrophic cardiomyopathy and non-obstructive hypertrophic cardiomyopathy were analyzed and compared. Results There was no significant difference in left ventricular end-diastolic diameter(LVIDd) and left ventricular end-systolic diameter(LVISd) between the study group and the control group(P>0.05). There were significant differences in the thickness of ventricular septum, the thickness of posterior wall of left ventricle and the anterior and posterior diameter of left atrium(P<0.05). The difference in arrhythmias between obstructive hypertrophic cardiomyopathy and non-obstructive hypertrophic cardiomyopathy was statistically significant(P<0.05). Obstructive hypertrophic cardiomyopathy was more prone to arrhythmia than non-obstructive hypertrophic cardiomyopathy, especially in atrial and ventricular arrhythmias. Conclusion Echocardiography is simple and noninvasive, which can be used to diagnose and type hypertrophic cardiomyopathy quickly and provide a rapid and accurate basis for clinical diagnosis and treatment.
